Health and safety failings led to hole in shin
Health and safety failures at a Workington company led to an employee suffering injuries to his leg.
Jamie Graham was left with a hole in his shin after a steel cable hit him.
Following an investigation by the Health and Safety Executive (HSE) into the incident, concrete panel producer ACP was fined £15,000.
Mr Graham needed to be freed by the fire and rescue service after the accident.
HSE inspector Mike Griffiths described the incident as "terrifying" and told how ACP did not have the correct systems in place for inspecting grips.
"The company should have ensured that the task of re-threading the cables was properly assessed and that the significant risks to their employees were properly controlled," he said.
Formica Ltd has also been fined this week after health and safety failures led to a worker losing his little finger.
The firm was told to pay £8,000 after the HSE deemed the company to be at fault for Gary Burke's injuries.
