Driver killed after health and safety failures
Health and safety failures have led to the death of a driver for a waste management company.
Gary Carter was crushed to death between his own lorry and a bulldozer when a lack of communication led to the driver of the compacter behind Mr Carter's vehicle pushing it forward into him.
Following an investigation by the
Health and Safety Executive (HSE), SITA UK Limited was prosecuted and pleaded guilty to breaching
health and safety regulations.
As a result, it was fined £210,000 and ordered to pay the full cost of the court case - £38,000.
HSE inspector Roy Bush said the company had not properly implemented
health and safety measures after bringing in new working conditions.
He said: "Employers need to ensure their staff understand their roles and responsibilities in making sure sites like this operate to clear site safety rules."
This comes shortly after Roadways Container Logistics Limited was fined £250,000 following the death of admin manager Alan Fletcher, who was crushed by a falling case of glass.
