Bin man died after health and safety failures
A London refuse collection company has been fined £130,000 and ordered to pay costs of £220,000 after
health and safety failures led to a man dying.
Veolia Environmental Services was prosecuted by the
Health and Safety Executive (HSE) after it was found to have breached
health and safety regulations.
David Ives, a refuse collector, was killed after a 1,100-litre recycling bin fell on his head.
Mr Ives had been clearing rubbish from outside a pub in Easington, near Aylesbury, when the accident happened on May 5th 2004.
Dennis MacWilliam, HSE inspector, said if the recycling lorry's bin hoist had been maintained properly, the incident would not have happened.
He said: "Employers are legally required to make sure their equipment is regularly maintained and is fit for use by their workers."
Richard Berry, executive director of Veolia Environmental Services, said the company deeply regretted the incident and had invested in training and equipment since Mr Ives's death.
